SIRS REPORTING FORM NOW AVAILABLE ON DIVISION WEBSITE

For those associations that must obtain a structural integrity reserve study (SIRS), the statute includes certain reporting requirements: 

  • Within 45 days after receiving the structural integrity reserve study, the association must distribute a copy of the study to each unit owner or deliver to each unit owner a notice that the completed study is available for inspection and copying upon a written request. Distribution of a copy of the study or notice must be made by United States mail or personal delivery to the mailing address, property address, or any other address of the owner provided to fulfill the association’s notice requirements under this chapter, or by electronic transmission to the e-mail address or facsimile number provided to fulfill the association’s notice requirements to unit owners who previously consented to receive notice by electronic transmission.

  • Within 45 days after receiving the structural integrity reserve study, the association must provide the division with a statement indicating that the study was completed and that the association provided or made available such study to each unit owner in accordance with this section. The statement must be provided to the division in the manner established by the division using a form posted on the division’s website.

The Division has now created the “Structural Integrity Reserve Study (SIRS) Reporting” form for reporting to the Division that the SIRS was completed, and that the association provided or made available such study to each unit owner.

Note that you will need your “project’s” license number and if you have multiple projects, you must submit a separate form for each project. The links below include instructions regarding how to find your license number.

There are three ways to submit the form to the Division:

  1. The association may complete an online form

  2. The association may complete the PDF form and email it to the Division at the following address: ctmhsirsreporting@myfloridalicense.com. For additional information on how to submit the form via email, please click here.

  3. The association may complete the PDF form and mail it to Division at the following address:

*NOTE: The online form asks the association to attach a copy of the SIRS to the report. However, this is not required by the statute. The PDF form created by the Division (which can be emailed or mailed to the Division) does not ask for a copy of the SIRS to be sent to the Division.
